After the death of a partner and father, three beneficiaries were denied $115,000 in life insurance benefits due to them.  Humana Insurance Company underpaid the claim based on two technicalities: (1) Humana claimed the decedent was not employed at the time of his death and thus his benefits were subject to a 50 percent reduction. (2) Humana claimed the decedent had not elected supplemental coverage. After two administrative appeals, Humana reversed its decision and paid the benefits in their entirety.
